ilbólgu af völdum sýkinga en í ristilsárabólgu
(13). Benda má þó á, að sjúklingar með
ristilsárabólgu geta einnig fengið Cam-
pylobacter sýkingar (12). Nauðsynlegt er
því að túlka niðurstöður úr vefjarannsókn í
ljósi allra klíniskra upplýsinga.
Iðrabólga af völdum C. jejuni er
sjúkdómur sem oftast læknast án meðferðar.
Sýkillinn er næmur fyrir mörgum
sýklalyfjum. Erýþrómýsín er heppilegast ef
beita þarf meðferð, en einnig má nefna te-
trasýklín, amínóglýkósíð, klindamýsín og
klóramfeníkól (17). Sýkilinn getur fundizt í
hægðum allt að 5 vikum eftir sýkingu (4, 17)
og því getur verið nauðsynlegt að beita
meðferð, þegar hætta er á að aðrir menn
sýkist. Nauðsynlegt er að fylgjast vel með
drykkjarvatni, ógerilsneyddri mjólk og
afurðum frá húsdýrum og gæludýrum, sem
geta borið sýkilinn, sérstaklega á það við um
hænsnfugla (4, 17).
Ein aðalhættan við þennan sjúkdóm er, að
hann sé rangt greindur og að það leiði til
rangrar meðferðar. Bæði klínísk einkenni og
vefjabreytingar líkjast ristilsárabólgu. Dæmi
eru til þess, að sjúklingar með Campylo-
bacter iðrabólgu hafi fengið salazópýrín og
sterainnhellingar í ristil. Slíkt getur bæði
tafið fyrir bata og leitt til hættulegra fylgi-
kvilla.
SUMMARY
ENTERCOLITIS DUE TO CAMPYLOBACTER
JEJUNI IN ICELAND.
The histopathology of rectal mucosa may resemble that
of ulcerative colitis.
Of 4019 fecal samples submitted to the Department of
Bacteriology University Hospital of Iceland during the
24 months from Oct. 1980 to Sept. 1982, Campylobacter
jejuni was grown from 67 (1.67%) using Skirrow’s-
medium. These patients were contacted during a pre-
vious study (5). Colonoscopy had been performed in
nine cases and rectal mucosal biopsies taken from five
patients. The clinical findings, colonoscopy findings
and the rectal mucosal histopathology findings are re-
ported. Patients may have bloody diarrhea, abdominal
pains, vomiting, fever and arthralgias. These symptoms
may be indistinguishable from those of ulcerative coli-
tis. The biopsies may show edema, lymphoid hyper-
plasia, inflammatory cell infiltration, crypt abscesses
and decrease in the number of goblet cells. In general
these changes are of a milder degree than in ulcerative
colitis. In three of these five cases the clinical diagnosis
had been inflammatory bowel disease or ulcerative co-
litis and in two cases the original pathology report had
confirmed the diagnosis of ulcerative colitis. Only two
cases were correctly diagnosed as infective colitis by
histology before the results of bacterial culture were
known. The necessity of proper laboratory examinati-
ons, including fecal culture and histopathology studies
are emphasized.
