líklegum sjúkdómsvöldum hjá þeim börnum, sem
fæðast eftir langan legvatnsleka og virðist á
þessum grundvelli ekki vera ástæða til að mæla
með sérstakri einangrun þessara barna.
Á sama hátt gefa niðurstöður okkar til kynna, að
gildi yfirborðsræktana við greiningu á innri
sýkingu sé takmarkað. Þó að meirihluti sýkta
hópsins hafi verið með hugsanlega
sjúkdómsvalda á húð, var engu að síður meira en
þriðjungur þessa hóps með neikvæða ræktun eða
sýkla, sem ekki eru sennilegir sjúkdómsvaldar.
Um þriðjungur ósýkta hópsins var með sýkla,
sem telja má líklega sjúkdómsvalda. Verður því
ekki séð, að niðurstöður yfirborðsræktana hafi
stuðlað að bættri greiningu sýkinga hjá þessum
hópi.
Það er þvi niðurstaða okkar, að verulega aukin
hætta á nýburasýkingum fylgi því, þegar legvatn
hefur farið lengur en sólarhring fyrir fæðingu
barns. Það er því mjög nauðsynlegt að fylgjast
sérstaklega með þessum börnum, einkum fyrstu
tvo sólarhringana og bregðast skjótt við, ef
barnið sýnir merki um sýkingu. Niðurstöður
okkar gefa ekki tilefni til að ætla, að þörf sé á að
einangra þessi börn frá öðrum nýburum í
sóttvarnarskyni.
SUMMARY
A retrospective survey was undertaken to evaluate the
effect of prolonged rupture of membranes (greater than
24 hours before delivery) on the health of the full term
newborn. Over a five year period, 117 infants were
admitted to the neonatal unit at Landspítalinn for
observation because of proionged rupture of
membranes. Of these, 14 were treated for an internal
infection, one with proven sepsis, seven with congenital
pneumonia and six with probable sepsis giving an
incidence of 12%. This is more than tenfold what is
expected. Surface cultures did not show bacterial flora
different from what has been reported in healthy
newborns. All infected infants were treated successfully.
It is concluded that newborns born after prolonged
rupture of membranes should be observed very closely
for signs of infection. Isolation of these infants does not
appear necessary.
