LÆKNABLAÐIÐ
Öfugt við áhrif klórals á svefnmynstur eru
lyfjahvörf þess all vel rannsökuð, þar á meðal
umbrot (4, 25, 26, 27). Varðandi umbrot
klórals vekur athygli, að etanól hvetur umbrot
klórals í þríklóretanól (28) (sbr. mynd 2). Er
það vegna þess, að við oxun á etanóli verður
aukið framboð á NADH, sem er hjáenzým
alkóhóldehýdrógenasa við afoxun á klórali í
þríklóretanól. Þar að auki hamla bæði klóral
og þríklóretanól umbrotum etanóls (28, 29).
Er því augljóst, að taka klórals og neysla
áfengis samtímis getur valdið alvarlegum
eitrunum.
Sú spuming hlýtur að vakna, hvort notkun
klórals og afbrigða þess eigi lengur nokkum
rétt á sér. Fljótt á litið mætti ætla, að klóral
væri úrelt svefnlyf, og svefnverkun þess er
raunar heldur illa rannsökuð eins og áður
segir. A hinn bóginn er vert að minnast
þess, að klóral umbrotnar ekki nema að litlu
leyti fyrir tilstilli míkrósómal enzýma (sbr.
mynd 2). Aðalumbrotsleið þríklóretanóls er
að vísu samtenging við glúkúrónsýru, en sú
umbrotsleið virðist ekki truflast neitt ámóta og
oxun í lifrarmíkrósómum, þegar gamalt fólk
á í hlut. Sést slík truflun vel, þegar umbrot
benzódíazepínsambanda, ekki síst tríazólams
og mídazólams eru skoðuð (30). Við emm því
hallir undir þá gömlu fullyrðingu, að klóral
gæti átt rétt á sér við lækningar í öldruðu
fólki. í öðrum tilvikum á lyfið ekki mikinn
rétt á sér.
Við getum að lokum ekki stillt okkur um að
nefna, að klóral er notað í umtalsverðum mæli
til dýralækninga hér á landi (súrdoðaskammtar
handa kúm innihalda 10 g af klóralhýdrati og
50 g af natríumbíkarbónati).
SUMMARY
During the period 1978-1987 altogether II cases of
fatal poisonings involving chloral were subjected to
forensic toxicological analysis in the Department of
Pharmacology, University of lceland. The results
are shown in the table. The concentrations of
trichloroethanol (the active metabolite of chloral)
were on an average highest in the liver and then in
brain, urine and blood in that order. The individual
results, however, varied within a wide range in
all tissues. Only two of these poisonings were
caused by chloral alone (cases no. 8 and 9). In
the nine remaining cases other drugs had also been
ingested (the table last column). These drugs were
considered contributory causes of death in at least
six cases (no. 3, 4, 5, 6, 10 and 11) and might
have been the most important causes of death in
two cases (no. 5 and 11).
In spite of the fact that chloral is little used as a
hypnotic in Iceland, fatal poisonings by the drug
are relatively common. Thus they accounted for
25% of all cases of fatal poisonings by hypnotic
drugs and 9% of all cases of fatal dmg poisonings
in the aforementioned period.
From this data it is concluded that the use of
chloral is largely unjustified as it has relatively
small therapeutic index and could easily be
replaced by safer hypnotic drugs. The only real
exception beeing the treatment of insomnia in the
elderly where impaired microsomal oxidation of the
benzodiazepines could justify the use of the drug.
