greinar og okkar eigin niðurstöðu, erum
við þeirrar skoðunar, að almennt þurfi
ekki að gera aðgerð á þöglum steinum. Við
álítum ennfremur, að sjúklingar þeir, sem
fá ódæmigerð einkenni (fl. II), einkum
þeir, sem eru eldri en 60 ára, þurfi ekki
nauðsynlega aðgerð, en ætti að uppfræða
þá hina sömu um möguleika á aðgerð síðar
og áhættu henni samfara, kynnu dæmigerð
(fl. I) einkenni að koma fram. Við teljum
ekki, að margir steinar í starfhæfri gall-
blöðru geri aðgerð nauðsynlega, þótt r.ðrir
séu á öndverðum meiði í þessu efni.18
Atriði, sem við teljum að styðji aðgerð,
jafnvel hjá þeim, sem hafa þögla gallsteina,
eru eftirfarandi: 1. Búseta á afskekktum
stöðum fjarri fullnægjandi læknisþjónustu.
2. Sjúklingar yngri en 40 ára og sennilega
einr.ig yngri en 50 ára. 3. Aðgerð, sem gerð
er vegna annarrar ástæðu þar sem viðbót-
ar gallaðgerð mundi ekki teljast auka að-
gerðaráihættuna á marktækan hátt. 4. Þeir,
sem hafa sykursýki,1 þó að okkar niður-
staða mundi ekki sérstaklega styðja val-
aðgerð í því tilviki. 5. Sjúklingar, sem fara
fram á valaðgerð eftir að þeim hafa verið
skýrðar allar staðreyndir.
Framtíðin kann að bera í skauti enn aðra
aðferð eða uppleysingu á gallsteinum með
efnafræðilegri aðferð, eins og nýlegar
greinar sanna.10 30 31 Þessi síðastnefnda að-
ferð verður þó enn að teljast á tilrauna-
stigi.30
SUMMARY
127 patients with completely silent gallstones
V-z developed colic and % non specific dyspepsia.
Those, who had multiple stones, disbetes, car-
diovascular disease and obesity did not demon-
strate increased risk of the development of
colic or dyspepsia. There was 0,7% mortality
and no morbidity related to gallbladder disease
or surgery. Patients above 61 year of age
showed less likelihood of future development
of colic. Both sexes showed the same chance of
contracting colic.
It is felt, that prophylactic cholecystectomy
for completely silent gallstones is unjustified
and not warranted in general. A few excep-
tions to this general rule are mentioned and
discussed.
