sem tetrasýklín er ekki sýkladrepandi heldur
sýklahemjandi. Yfirleitt var sýklalyfjagjöf
hafin of snemma og haldið áfram of lengi, en
það átti einnig við í fyrri skosku könnuninni
(16). Notkun sýklalyfja hjá tannlæknum
í Skotlandi hefur batnað til muna eftir að
síðustu tilmæli voru gefin út í Bretlandi, en
athyglisvert er að innan við 30% íslenskra
tannlækna fylgja þeim tilmælum sem gefin
eru út hér á landi (15). Breyting á tilmælunum
yfir í einskammtagjöf amoxýsillíns, sem mælt
er með af mörgum hjartalæknafélögum (14),
gæti bætt sýklalyfjanotkun hjá tannlæknum
hér á landi eins og gerðist í Skotlandi. Þar
styttist mikið tíminn sem sýklalyf eru gefin
(16). Nýlega hefur tilmælunum verið breytt í
þessa veru í Svíþjóð (17).
A íslandi fer tannheilbrigði vaxandi og
einstaklingamir koma þess vegna til með
að halda tönnum sínum lengur en áður (18).
Þetta leiðir til þess að fleiri eldri einstaklingar
þarfnast tannaðgerða, en þeir eru mun
líklegri til að vera í áhættuhópi varðandi
hjartaþelsbólgu. í Skotlandi hefur komið í ljós
að ástand tanna einstaklinga í áhættuhópum
fyrir hjartaþelsbólgu er slæmt (19), og álitið
er að ástandið á íslandi sé síst betra.
Þessi könnun hefur leitt í ljós að átaks er
þörf til þess að bæta sýklalyfjagjöf tannlækna
í forvamarskyni. Þörf er á aukinni fræðslu,
einnig má ætla að aukin samvinna tannlækna
og hjartalækna annars vegar og tannlækna og
sjúklinga hins vegar geti bætt ástandið.
SUMMARY
The use by Icelandic dentists of prophylactic
antibiotics for patients at risk of developing
infective endocarditis was investigated in a
questionnaire sent to all dentists in the country.
Only 68 of 204 dentists (33%) replied of whom
57% had used prophylactic antibiotics in the
preceding 6 months. Penicillin was the antibiotic
of first choice but there was little consensus
among respondents about the timing of prophylaxis
and few dentists actually followed the Icelandic
recommendations. The results of the questionnaire
were broadly in agreement with those obtained
from a similar survey carried out in Scotland
in 1981. A great improvement in usage of
prophylactic antibiotic was witnessed in Scotland
in 1985 after the introduction of amoxycillin as a
prophylactic antibiotic for dental use. The present
study in Iceland suggests the need for a change in
recommendations for prophylactic procedures to
more practical regimens such as have now been
widely adopted in Europe.
