Aim: rural emergency departments are designed to provide short-
term emergency care for seriously ill or injured patients. The pur-
pose of this study was to explore rural emergency nurses’ self-
assessment of competence.
Method: This descriptive cross-sectional study was conducted in
the spring of 2016. a survey, including the nurse Competence
Scale (nCS), was distributed to 87 icelandic rural nurses who pro-
vided emergency care to a minimum of ten acutely ill or injured
patients each month. The nSC scale contains 73 questions in seven
nursing domains. The return rate was 60%. The data was analysed
using descriptive and inferential statistics.
Results: The nurses assessed their competence highest, and their
frequency of actions most, in the category managing situations.
They assessed their competency lowest in ensuring quality and their
frequency of actions least in teaching-coaching. Looking at specific
competency items, they assessed their competence highest, and
their frequency of actions most, in acting autonomously. nurses
with more than five years of work experience assessed their com-
petence significantly higher in five domains (managing situations,
work role, diagnostic functions, therapeutic interventions and teach-
ing-coaching) than those with shorter work experience. nurses with
post graduate education in nursing, assessed their competence sig-
nificantly higher in all competency categories than nurses without
post graduate education.
Conclusion: The findings indicate that post graduate education
and work experience influence the self-perceived competence of
nurses who provide emergency care in rural settings. administra-
tors of health institutions should enable nurses to seek further ed-
ucation and attempt to hold on to those with long work experience.
