LÆKNABLAÐIÐ 1997; 83 107 erfiður vegna þess hversu tilfelli eru fá og því erfitt að leggja mat á árangur. í okkar tilfelli var bæði gerð gangaaðgerð Takeuchis og hefð- bundin hjáveituaðgerð með vinstri brjósthols- slagæð. Okkur er ekki kunnugt um hvort báðar þessar aðgerðir hafi verið notaðar samtímis áður. Með hjáveitunni er blóðflæði til vinstri slegils tryggt til frambúðar ef göngin stíflast til dæmis vegna æðakölkunar síðar á ævinni. Göngin er hægt að gera hæfilega víð og ná- kvæmlega í samræmi við höfuðstofn vinstri kransæðar. Göngin geta því flutt allt það blóð sem vinstri slegill þarfnast við mikla áreynslu. Við teljum góð rök fyrir því að gera bæði ganga- og hjáveituaðgerð hjá sjúklingum með meðfædd upptök vinstri kransæðar frá megin- lungnaslagæð. Sérstaklega á það við ef um skaddaðan vinstri slegil er að ræða eins og í ofangreindu tilfelli þar sem gert var við hjartað með því að nema burtu gúl af vinstri slegli og koma á þriggja æða kransæðakerfi í stað eins illa starfandi.
