Tilgangur þessarar rannsóknar var að kanna
þjónustu við veik börn og framkvæmd IMCI í lág-
tekjulandi í Afríku sunnan Sahara. Niðurstöður
hennar sýna að þetta alþjóðlega átak, byggt á
vísindalegum niðurstöðum, nær til heilsugæslu á
landsbyggðinni í fátæku landi eins og Malaví. Þær
sýna einnig að IMCI nálgunin er heppileg og tekur
á langflestum sjúkdómum sem hrjá börn í umhverfi
sem Monkey Bay. Einnig er ánægjulegt að stað-
reyna að skipuleg eftirfylgd fer fram á þekkingu
starfsmanna á IMCI og framkvæmd þeirra á því og
að nauðsynlegustu lyf eru oftast aðgengileg. Aftur
á móti er flokkun sjúkdóma mismunandi eftir heil-
brigðisstofnunum, greining öndunarfærasýkinga
ómarkviss og hugsanlegt að vannæring og langvar-
andi niðurgangur séu vangreind vandamál. Það er
áhyggjuefni að þjónustugjöld virðast samkvæmt
þeim niðurstöðum sem hér eru kynntar hafa veru-
leg áhrif á það hvert menn sækja heilbrigðisþjón-
ustu. Reynsla nágrannaþjóða sýnir að það eru þeir
fátækustu meðal fátækra sem sitja heima og líklegt
að svo sé einnig á Monkey Bay svæðinu. Því þurfa
stjórnvöld að efla grunnnet þjónustunnar og gera
hana aðgengilega fyrir alla landsmenn án tillits
til efnahags. Til að svo megi verða og að nokkur
von sé um að þúsaldarmarkmiðin náist fyrir þá
sem eru búsettir í Malaví og öðrum Afríkuríkjum
þarf alþjóðasamfélagið að bindast böndum með
stjórnvöldum í því að efla og styrkja heilsugæsluna
með viðeigandi þróunaraðstoð.
