Langvinn lungnateppa og aldraðir
Samfella í meðferð einstaklinga með LLT er mikilvæg.
Þessir einstaklingar hafa í flestum tilfellum notið sérhæfð-
rar hjúkrunarþjónustu árabil áður en þeir koma á hjúkr-
unarheimili eða fara að nota þjónustu heimahjúkrunar.
Eftirfylgd, ráðgjöf og samvinna milli fagstétta er því
nauðsynleg til að einstaklingurinn haldi áfram að fá bestu
þjónustu sem í boði er.
