— Artist drawings of two liypotheses regarding etiology.
Figure 1.
A. Pyloric obstruction: Air proximal to it
gces through the mucosa and dissects
along the bowel wall or down the
mesentery to the colon.
B. Rupture of alveoli in chronic lung
diease and subsequently air is forced
along vessels and brounchi into media-
stinum. There is then leakage trough
the diaphragm along major vessels and
their splanchnic branches and finally
entrance into the bowel wall along thc
smaller vessels that perforate it.
Þriðja hugmyndin um orsök er áverki,
til dæmis við ristilspeglun, þegar sigmoid
hluti ristils er blásinn upp við þá rann-
sókn. Greint hefur verið frá loftblöðrum
í þörmum eftir garnatengingar, einkum
þarmastyttingar. Hiá Feinberg(4) reynd-
ust 16.2% er gengist höfðu undir þarma-
styttingu, cg röntgenmynd var til af eftir
uppskurð, vera með loft í þarmavegg-
Fjórða hugmyndin er að þarmasýklar
framleiði loft í þarmaveggnum. Sýnt hefur
verið fram á loftmyndun í þarmavegg til-
raunadýra eftir inndælingu af Clostridium
perfringens(14).
MEÐFERÐ.
Allt fram til 1970 var meðferðin fyrst
og fremst symptomatisk, en skurðaðgerð í
svæsnustu tilfellum. Eins og sjá má af töflu
I innihalda blöðrurnar mikið af köfnunar-
efni og koldioxið, en lítið súrefni. Sú hug-
mynd kom fram 1973(5), að með auknum
deiliþrýstingi súrefnis í blóði, mætti fá
aðrar lofttegundir til að flæða úr blöðrun-
