foreldrahlutverki geti skipt máli (Halldóra Ólafsdóttir, 1998;
Cooper og Murray, 1997; Appleby og fl., 1997; Holden og
fl., 1989). Nýorðnar mæður, sem líður illa, virðast hafa þörf
fyrir eins konar uppeldis- og foreldraskóla sem myndi veita
aukna aðstoð við umönnun ungbarna ásamt leiðsögn í
samskiptum við barnið, sérstaklega við erfið ungbörn
(Thome, 1996, 1992). Ýmiss konar heilsufarsvandamál
kvennanna og annarra í fjölskyldunni voru einnig nefnd
sem algeng ástæða fyrir vanlíðan mæðra, og það undir-
strikar mikilvægi heilsu- og mæðraverndar eftir barnsburð.
Ályktun
Af niðurstöðum þessarar rannsóknar er ályktað að mikil og
langvarandi vanlíðan íslenskra kvenna eftir barnsburð sé
sjaldan greind og meðhöndluð að frumkvæði heilbrigðis-
stétta og að konurnar leiti í fáum tilvikum eftir aðstoð
fagstétta. Sumar telja sig of veikar til að sinna athöfnum
daglegs lífs eða til að hafa ánægju af umönnun ung-
barnsins. Flestar virðast bíða eftir aðstoð en sumar eru
ekki nægilega virkar til að leita eftir henni. Til stuðnings
þessari ályktun er vitnað orðrétt í móður sem býr í
afskekktri sveit og átti ekki kost á faglegri meðferð sökum
frumkvæðisleysis sem er eitt af einkennum þunglyndis:
„Við erum búin að byggja upp ágæta mæðravernd um
allt land - en hvað um okkur sem líður svo illa að við get-
um á köflum varla sinnt sjálfum okkur né börnum okkar?“
Tillaga
Lagt er til að íslenskar heilbrigðisstéttir, sem fást við
umönnun mæðra og ungbarna, taki frumkvæði í að leita
uppi konur sem þjást af vanlíðan eftir barnsburð og veiti
þeim sem líður illa viðunandi meðferð.

Abstract:
Mothers of Difficult Infants:
What health care do they receive for depression and
parent stress?
A follow-up study.
Objective: The aim of the study is to explore the
perception of mothers with a difficult infant on the health
care they received postpartum whilst they experienced
frequent and long-lasting depressive symptoms and a
high degree of parental stress.
Material and methods: The well-being of mothers and
infants was investigated in a cross-sectional national survey
in 1993. A sample of all lcelandic women who gave birth
during a quarter of the year 1992 and had a live baby two

months later was selected from the National Register
(N=1053). The population was lcelandic mothers who had
given birth in 1992 (N=4591). Mothers with a difficult infant
(assessed by an Infant difficulty index) were selected from
sample respondents on preset scores of two distress-
measures: Edinburgh Postnatal Depression Scale (EPDS)
and Parent Stress Index / Short Form (PSI/SF).
Unexpectedly, a sub-sample of severely distressed mothers
emerged during the selection process and was followed up
by half-structured telephone interviews.
Results: Thirty-seven women (5%) of the surveyed
population were identified as severely distressed. They
validated their experience of distress in the early post-
partum by telephone interviews. About half of them
reported some improvement two months later. The other
half felt no improvement or even worsening. Only one
woman out of four had received health care for distress by
various professionals. Very few women utilized the service
available at Health Care Centers and mentioned a variety of
reasons for this. It is concluded that a low proportion of
postpartum severely distressed lcelandic mothers is
detected and treated by health professionals and that these
women show little initiative to seek professional care.
