hjartaútfalli en við teljum líklegt að hún sé
röng enda urn stóran fistil að ræða. Auk þess
lögðuðust mæðin og hækkaður bláæðaþrýst-
ingur á hálsi eftir aðgerðina.
Talið er að flestir A-V fistlar á portæðar-
svæði valdi fyrr eða síðar einkennum og því er
rétt að loka þeim. Hægt er að uppræta þá með
opinni aðgerð líkt og gert var í þessu tilfelli en
einnig kemur til greina að notast við „embólí-
seringu“, sérstaklega ef erfitt er að komast að
fistlinum. Þeirri aðferð hefur aðallega verið
beitt við A-V fistla á portæðarsvæði eftir
áverka og rof á slagæðagúlum (19,24-26) og
eru yfirleitt notaðir til þess sérstakir stálvírar
sem komið er fyrir í fistlinum við slagæða-
myndatöku og stífla hann (25). Ekki var talið
fýsilegt að beita „embólíseringu1' í ofangreindu
tilfelli, fyrst og fremst vegna stærðar fistilsins.
Hins vegar má geta þess að „embólíseringu"
var nýlega beitt við tvo sjúklinga á Landspítal-
anum með góðum árangri en þeir höfðu báðir
A-V fistla í lifur eftir lifrarástungu.
