LÆKNABLAÐIÐ 1996; 82 275 betri árangri með segaleysandi lyfjum en tókst í þeint rannsóknum sem báru saman segaleys- ingu og víkkanir, til dæmis með hraðari gjöf tPA (16,17). Ef til vill má einnig bæta árangur- inn með betri blóðþynningu í kjölfar segaleys- andi meðferðar. í báðum tilvikum er líklegt að að minnsta kosti hluti slíks ávinnings tapist aftur vegna aukinnar blæðingarhættu. Það er ljóst að í þessum efnum eru ekki öll kurl komin til grafar og verður spennandi á næstunni að fylgjast með hvort línur skýrast. Niðurstaða Reynslan af fyrstu fimm sjúklingunum sem gengust undir tafarlausa kransæðavíkkun á ís- landi bendir til að unnt sé að veita slíka með- ferð hérlendis með góðum árangri. Meðferðin er á margan hátt rökrétt. Æðin er opnuð hratt og örugglega með lágmarks blæðingarhættu og greining og lækning eru afgreiddar í einni sjúkrahúslegu. Þeir sem líklegastir eru til að hafa gagn af slíkri meðferð og fá kransæða- stíflu í dagvinnutíma ættu að eiga möguleika á að fara í tafarlausa kransæðavíkkun þótt hinir sem fá kransæðastíflu að kvöldi eða að nætur- þeli geti ekki notið kosta hennar enn.
